Thank you for completing the Initial Enquiry Form.
Our Case Managers will use the information you provided to form the basis of your Debt Agreement. A copy of your submissions will have been emailed to the address you provided. If your circumstances change, please call your Case Manager and let them know.
The Final Step
In order to prepare your Debt Agreement, we will also need you to supply a number of supporting documents. These are:
- Copy of driver’s license or proof of age card
- Copy of passport
- Copy of Medicare card
- Pay slips (2 most recent)
- Child support statement (if applicable)
- Centrelink statements (if applicable)
- Tax return and/or group certificate
- Recent statements and letters of collection on all loans and debts (including secured). Full disclosure of all debts is very important.
- Superannuation statement

You can supply these documents to us in a number of ways.
Scan your documents and email them to your Case Manager on their personal email address.
